Insurance

Emily Kerlin is currently accepting Aetna only at this time

Mary Ann Green is accepting some various insurance plans:

She is currently in network with Aetna, Optum and United Healthcare and affiliated plans for therapy, but not nutrition counseling. To check your eligibility, please go to: https://secure.helloalma.com/providers/mary-green/ 

If your insurance is not accepted, we are happy to provide a superbill for you to submit to your insurance company for out-of-network reimbursement.  If you would like to do this, we recommend asking your insurance company the following questions:

  • How much money will I be reimbursed for an individual mental health session (code # 90834 or #90837) with an out-of-network provider?
  • If they cover a percentage of the “Usual and Customary rate,” ask what that looks like in dollars.
  • What exact paperwork do you need from me (the client) in order to receive my reimbursement?

*It is important for you to know that in order to use mental health insurance benefits, your insurance company will require the provider to submit a diagnostic code representing one of the mental/behavioral disorders found in the DSM-5 diagnostic manual and may require access to your confidential medical records. This could affect future employment and health/life insurance premiums, and court/legal issues.

If you pay out of pocket, submitting diagnoses to insurance companies is not necessary.

Payment

Payment is due at the time of service.  We accept cash, check and all major credit cards as forms of payment.  We can also accept Health Savings Account cards.

Cancellation Policy

We do have a 24 hours cancellation policy and ask our clients to give us 24 hours notice when needing to cancel a session. We do understand that life happens, and exceptions will be made in cases of emergency or illness, however, we must charge a fee for no shows and late cancellations as there was a spot that could have been filled with proper notice. The earlier you can give notice, there is a higher chance for a spot being available to another client. Please read below for further clarification on our cancellation fees:

  • No Shows – No shows will be charged a $100 fee, only illness or emergencies are exceptions.
  • Late cancellations – If able to reschedule within the same week, we will waive the fee.  If unable to reschedule within the same week you will be charged $50 for the late cancellation.
  • True illnesses and emergencies will continue to be excused.
